RX Care Club Prescription Plan (Care Card Member benefit only)*
- With the Care Card Plus, Customers are eligible to receive discounts on over 250 select generic medications.
- This card does not constitute insurance and is not intended as a substitute for insurance.
- The RX Care Club Prescription Plan may not be used in coordination with any other prescription coverage.
- Pricing will be calculated on a days supply basis at commonly prescribed dosages. For example, a 30-day supply of medication will cost $4 or $9 depending on the drug. A 90-day supply of medication will cost $10 or $24 depending on the drug.
- St. Marys Pharmacy, Inc. reserves the right to define the commonly prescribed dosages.
- St. Marys Pharmacy, Inc. reserves the right to make additions or deletions from the generic drug program at any time without cardholder notification.
- Persons receiving benefits from publicly funded health care programs are ineligible to receive a discount under the RX Care Club Prescription Plan.
- Prepackaged drugs are covered only in unit sized specified on drug list.
- Prescriber permission is required to change a 30-day prescription to a 90-day prescription.
- Drugs included in St. Marys Pharmacy, Inc.'s generic drug program are not intended as a recommendation of therapy or a substitute for consultation with your physician about a proper drug therapy for you.
- The RX Care Club Prescription Plan Details are subject to change at any time without cardholder notification.
The Care Card Rebate and Sales Prices*
- The Care Card also entitles customers of St. Marys Pharmacy, Inc. to receive a minimum of 1% rebate on most purchases made at St. Marys Pharmacy or SMP Pharmacy & Home Medical Kane. Rebate does not apply to the following purchases: tobacco, lottery,dairy products, home medical supplies, JCPenney purchases, phone cards, or gift cards.
- The rebate will be loaded onto your Care Card and may only be used on future purchases made at the aforementioned stores. The rebate cannot be applied to any prior purchase.
- The rebate cannot be redeemed for cash.
- Care Card holders will also receive sale prices throughout the store as well as other specials including:
- 10% off most vitamins
- Double points on select Quality Choice Brand products
- Point balances are displayed on the bottom of all cash register receipts.
